During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Today we discussed program leader qualification, standard 8VAC20-780-210. A copy of a qualification grided was emailed to the program director.

An unannounced, on-site monitoring inspection was initiated on and completed on 9/15/2025, as a part of the licensure period. The on-site inspection began at 4:30pm and ended at 6:30pm. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 staff. The inspector reviewed 5 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 9/26/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-M Description: The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan as required. This list shall be dated and kept confidential in each room or area where children are present. The allergy list containing names of children and their specific allergies, sensitivities and dietary restrictions was posted in such a way that anyone could read the information. Standard #: 8VAC20-780-270-A Description: Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.
1 - There is chipped area of an activity table in the toddler room that has a jagged edge which is accessible to children in care.
2 ? The glass shower door in the bathroom on the second floor is hanging off the track and appears to be a toppling hazard.Plan of Correction: ? The activity table was removed
immediately; a replacement has been
ordered.

Standard #: 8VAC20-780-430-K Description: Provision shall be made for an individual place for each child's personal belongings. There were several children?s blankets stored on an open shelf. There is not a individual place for each child?s belongings. Plan of Correction: ? All blankets removed from shared
shelf.

been assigned to each child.
? Staff instructed to ensure belongings
are placed in assigned space daily.

before bringing to center.
