An unannounced inspection was completed on 01/10/2025 in response to a complaint received by the licensing office on 01/09/2025 relating to allegations of administration. This inspection included document review (policies and procedures) and interviews.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/01/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Complaint related: Yes Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards. The center failed to notify the parent of C1 of an injury when it occurred on 10/24/2024. The injury was observed by the parent when the child was leaving the center for the day. S1 acknowledged not notifying the parent of C1 of the injury and the provision of first aid when it occurred. Center policy requires that parents be contacted about any injury more serious than a small scrape. Plan of Correction: Staff was coached verbally in the moment. Management will notify parents as incidents occur.
Standard #: 8VAC20-780-60-A Complaint related: Yes Description: Each center shall maintain and keep at the center a separate record for each child. The center was unable to locate C1's file and injury report during the inspection and C1 had been enrolled for over 4 months. Plan of Correction: Child files are being reviewed and maintained.
