During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced monitoring inspection was initiated on 07/30/2025 and completed on 09/02/2025 in response to a self-report received by the licensing office on 07/07/2025 relating to supervision. The inspector reviewed five children?s records and six staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/10/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-40-E
Description: The operational responsibilities of the licensee shall include ensuring that the center's services are maintained in compliance with the center's own policies and procedures that are required by these standards. The center's playground policy states ?Children will be kept within sight and sound at all times. The center was in violation of their policy when interviewed staff confirmed Child #1 and Child #2 (three years old) were left unattended on the playground for approximately five to ten minutes on 06/06/2025.

Plan of Correction: The playground policy was reviewed and changes made due to the incident on June 6th, 2025. A review was done with all employees and changes implemented.

Teachers are required to take the roll out on the playground. Children are using a walking rope and are counted before leaving the building, after arriving at the playground. Then counted again before leaving the playground. A teacher sweeps the playground to ensure all children are indeed on the walking rope. The children are again counted after arriving in the building and checked to the roll.

Standard #: 8VAC20-780-340-A
Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.
Staff interviewed on 07/30/2025, staff confirmed Child #1 and Child #2 (three years of age), were left unsupervised on the outdoor playground for approximately five to 10 minutes on 06/06/2025. Staff stated the two children were inside a pirate ship located on the playground. The fenced playground is not adjacent to the center. A parking lot separates the child day center and fenced playground.

Plan of Correction: Two children were written up for not keeping two children within their sight and sound or for counting the children before leaving the playground. Two children were hiding in a pirate ship and were left on a fenced in playground without supervision for 5-10 minutes. As a result, teacher are required to take the roll out on the playground. Teachers are required to walk around on the playground to keep all children in sight and sound. Children are using a walking rope and are counted before leaving the playground. A teacher sweeps the playground to ensure all children are indeed on the waling rope. The children are again counted after arriving in the building and checked to the roll.

Standard #: 8VAC20-780-340-F
Description: Children under 10 years of age shall always be within actual sight and sound supervision of staff. On 06/06/2025, Child #1 and Child #2 (both three years old) were left unsupervised on the outdoor playground for approximately five to 10 minutes. Staff interviewed on 07/30/2025, confirmed that two children were on the playground without adult supervision for approximately five to 10 minutes.

Plan of Correction: All employees, present and new, will take a resource on Active Supervision presented by National Center on Health, Behavioral Health, and Safety. This includes, Setup the Environment, Position Staff, Scan and Count, Listen, Anticipated Children?s Behavior and Engage and Redirect. This is with the Office of Head Start. There is a written response to be turned in.

Planning for Positive Guidance: Powerful Interactions Make a Difference is another resource put out by naeyc that we are using to teach the ladies about guidance for infants, toddlers and preschoolers.
All of the above have been added to orientation to be completed within 30 days of hire.
