During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical assistance was provided and licensing standards were reviewed and discussed regarding required posting in facility

Comments:
An unannounced, on-site renewal inspection was initiated and completed on September 18, 2025. The on-site inspection began at 8:30am and ended at 10:45am. The inspector reviewed compliance in the areas listed above. There were 7 children present and 3 staff. The inspector reviewed 7 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-A
Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #5 has been enrolled at the center for approximately 3 weeks and did not have a copy of their immunization records in their file.

Plan of Correction: The center responded with the following: The owner called the child?s mother during the time of the inspection and sent to immunization and physical via email to the center.

Standard #: 8VAC20-780-130-E
Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Two children under the age of 2 years old did not have additional immunizations once every six months.

Plan of Correction: The center responded with the following: The owner will audit the files to ensure that all children under the age of 2 years old have the additional immunization records and should they not, contact the parents and request a copy for the record.

Standard #: 8VAC20-780-60-A
Description: Repeat Violation.
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the work phone number and a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Four of the seven children?s records reviewed did not have parent work numbers listed. Child #5 has a diagnosed allergy and there was no allergy action plan should the child have a suspected or confirmed allergic reaction.

Plan of Correction: The center responded with the following: The center will obtain all the work phone numbers for the children at pick up and will also email the parents to ensure that all contact information for places of employment are listed. During the time of the inspection, the owner called Child #5?s mother and requested documentation from the child?s physician regarding the diagnosed allergy and steps to take should there be an allergic reaction.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation.
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. In the outdoor playground a bike seat, play table and framing of a plastic playhouse had several cracks and breaks on the plastic which pose as an injury and pinching hazard.

Plan of Correction: The center responded with the following: The outdoor play equipment will be reinforced and/or repaired to ensure that the children are not injured by the equipment. Staff will examine the equipment outdoors routinely to ensure there are no cracks and broken pieces of equipment.

Standard #: 8VAC20-780-570-A
Description: When a child is placed in an infant seat the protective belt shall be fastened securely. Child #2 was in the highchair without the protective belt fastened.

Plan of Correction: The center responded with the following: The director fastened the child?s protective belt after being informed it was not fastened. Staff will be reminded to ensure that when a child is placed in a high chair, they will be fastened appropriately.

Standard #: 8VAC20-780-580-B
Description: Any vehicle used by the center for the transportation of children shall all meet the safety standards set by the Department of Motor Vehicles. That transportation van that is currently being used to take children to and from school has a State Inspection that expired in July 2025.

Plan of Correction: The center responded with the following: The center is aware that the State Inspection is expired and has a scheduled appointment to have the transportation van inspected on September 22, 2025.
