During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on 3929/25 and completed on the same day, as a part of licensure period. The on-site inspection began at 10:45 and ended at 12:00. The inspector reviewed compliance in the areas listed above. There were 13 children present and 3 staff. The school age program had 5 children with 1 staff The inspector reviewed 5 children?s records and 3 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Staff that resides out of state must have child abuse and neglect as well as sex offender searches conducted.
The center has two staff that are missing out of state searches or part of the searches.
Staff 2 is missing the sex offender search
Staff 3 is missing both searches.Plan of Correction: I will get the background checks needed, I think I did some already.
Standard #: 8VAC20-780-150-B Description: Each child must have a physical examination signed by a physician or an official from the health department.
One child, Child 2, is missing the signature of the physician. and the results of teh physical on the form.Plan of Correction: I will ask the parent for the information.
