During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site inspection was initiated on September 29, 2025 completed on September 29, 2025 in response to a complaint received by the licensing office on September 26, 2025 relating to allegations of Staffing and Supervision. The inspector reviewed one staff record on-site. This inspection included document review, interviews and observation.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
