During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated on 07/30/2025 and completed on 09/02/2025 in response to a complaint received by the licensing office on 07/30/2025 relating to supervision. The inspector was onsite from 10:00am until 2:00pm. The inspector reviewed five children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/15/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-A Complaint related: Yes Description: When staff are supervising children, they shall always ensure their care, protection, and guidance. On 07/28/2025, from 8:40am to 10:15am, Classroom #1 exceeded the required staff to child ratio, with two staff members supervising up to 30 children between the ages of 3 and 5. Two children in Classroom #1 had diagnosed special needs, and their records included assessments and behavior guidance strategies for staff. Staff reported that managing the number of children during this time frame was challenging due to instances of aggressive behavior exhibited towards other children, staff and self on a regular basis. Plan of Correction: Another full time teacher was hired, and we are looking to hire one more.
Also, we do not allow more than one employee off on the same day anymore so we can move ladies into open spots if a teacher is coming in late or calls out.
One child with special needs, undiagnosed, and violent is no longer with us. His parents were not seeking help for him. We felt that we were doing the child a disservice by not putting his parents in a position of needing to seek professional diagnosis. But 3 children needing diagnosis in the preschool program are no longer enrolled with us for the same and reason and 1 diagnosed is now in a program with the school system.
Standard #: 8VAC20-780-350-B-4 Complaint related: No Description: The staff-to-children ratio is one staff member for every ten children (age 3 years old up to school age eligible) in care. On 07/28/2025, Staff #1 and Staff #2 were out of ratio from 8:40am until 10:15am in Classroom #1. When Staff #3 arrived at 10:15am 30 children were present with Staff #1 and Staff #2. The children in the classroom were three years to five years old. Plan of Correction: Another full time teacher was hired, and we are looking to hire one more.
