During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
* WATER TESTING
Effective January 1, 2025, the VDOE began determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. * HEALTH INSPECTIONS
In collaboration with the Virginia Department of Health (VDH), the VDOE is reminding providers that the Food Establishment Health form is only used by VDH when determining compliance with food regulations.

A Report of Environmental Sanitation Inspection form should be used for environmental health inspections and determining compliance with non-food related health requirements such as water supply and sewage disposal. The Report of Environmental Sanitation Inspection Form can be found on the Department?s website.

Please request that your Health Inspector provide BOTH reports if your program provides food service.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 8/22/2025. The inspector reviewed compliance in the areas listed above. There were 16 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site on 8/22/2025. This inspection included document review, tour of the facility, interviews, observations and measurements

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: Staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department,
Staff #5's last TB screening was overdue by more than 1 year and 5 months.

Plan of Correction: Staff #5 will ensure that a TB screening is completed and that results are documented placed in her file within 2 weeks.

Standard #: 8VAC20-780-420-E-3
Description: Staff must request at least annually parent confirmation that the required information in the child's record is up to date. The records for child #2 and child #4 did not contain documentation to demonstrate that the parent had confirmed that the information in the child's record was up to date. This information was overdue by more than 7 months for child #2 and overdue by more than 2 years for child #4.

Plan of Correction: The licensee stated that this information will documented in child #2 and child #4's records by next week and annually all children's records will be reviewed for any needed updates and documentation of the annual review will be kept in the children's records.

Standard #: 8VAC20-780-560-F
Description: When centers choose to provide meals or snack, they are required to follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
USDA requires that require milk, grains or bread, meat or meat alternate, and
two different servings of fruit or vegetable be served at lunch. The children were served Chicken, green beans and apple juice for lunch. A grain and milk were missing from the required food components.

Plan of Correction: The licensee stated that beginning next week, the menu will be adjusted to ensure that USDA requirements are met at each snack and meal served to children.
