During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced inspection was initiated and completed on 10/1/2025 in response to a self reported incident received by the licensing office on 8/4/2025 relating to allegations of staffing, supervision and forbidden action. This inspection included:

? interviews, and
? observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
