An unannounced on-site focused inspection was initiated on 09/30/2025 and completed on 10/02/2025. The inspection included document review, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-80-B-2 Description: The center shall inform the department?s representative as soon as possible, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.
Child 1 (age 3 years) received an injury that required outside medical attention on 09/17/2025; the center did not inform the department?s representative within two business days of the injury that required outside medical attention.Plan of Correction: We now know to report immediately when a child is injured and requires outside medical treatment.
