During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced monitoring inspection was initiated on September 19, 2025 and completed on October 2, 2025. The inspector reviewed 1 child?s records and 2 staff records on-site on September 19, 2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to (area or part of standard) and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 20, 2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-40-E
Description: The operational responsibilities of the licensee shall include ensuring that the center?s activities, services, and facilities are maintained in compliance with these standards, the standards own policies and procedures that are required by these standards, and terms of the current license issued by the department. Child # 1 was left alone and unattended in the classroom for approximately 2 minutes when the staff did not follow center procedures and took the remaining children from the class outside to the playground.

Plan of Correction: Tierra Encantada ensures full compliance with licensing standards and internal policies. After the incident where one child was left unattended for about two minutes,
immediate retraining and safety reviews were completed. Management conducts daily face-to name audits, teachers perform checks every 30 minutes, and floating
staff support supervision to maintain child safety at all times

Standard #: 8VAC20-780-340-F
Description: Children under 10 years of age shall always be within actual sight and sound supervision of staff. On September 15, 2025, at approximately 3:25 pm, eleven, two year old children were taken outside to the playground by two staff, leaving 1 child alone on the carpet in the YPB room for approximately two minutes. Staff 1 and 2 acknowledged they did not do name to face transitioning to the playground.

Plan of Correction: All cot sheets are now in use in each classroom as required, and we continue to reinforce this practice with our staff members to ensure compliance during naptime.

Standard #: 8VAC20-780-450-A
Description: Cribs, cots, mats and beds used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. There were no linens available during naptime for 11 cots.

Plan of Correction: Tierra Encantada has established supervision and face-to-name procedures to ensure constant sight and sound supervision. Following the September 15, 2025
incident, all staff were retrained on supervision and transition protocols, and policies were reviewed and staff to reinforce safety and accountability.
