Comments:
An unannounced monitoring inspection was initiated and completed on 9/29/2025, as part of the licensure period. The on-site inspection began at 9:39am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 33 kids and 10 staff present. The inspector viewed on-site records of 4 children and 4 staff. Medication reviewed on-site on 9/29/25.

The inspection included document reviews, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-440-F
Description: If rest mats are used, they shall have cushioning and be sanitized on all sides weekly or before use by another child.

Multiple rest mats within the sites classrooms were observed to be punctured in a manner where the cushion was exposed. Since they are in bad repair, the rest mats are not able to be properly sanitized.

Plan of Correction: We corrected the nap mats. I had extra in the closet to take care of that immediately.
