An unannounced, on-site renewal inspection was completed on 09/30/2025, as a part of the conditional licensure period. The on-site inspection began at 1:45 pm and ended at 2:45 pm. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-3 Description: The center must request a search of the central registry prior to the employee's first day of employment.
Staff #3, who was working during this inspection, did not have a central registry search requested.Plan of Correction: Center will request Central Registry search for staff #3.
Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all of the required information.
The records for child #1, 2, and 3 did not contain the two required emergency contacts. The records for child #1-5 did not contain verification of proof of age and identification.Plan of Correction: Center will request information from parents for children's records.
Standard #: 8VAC20-780-70 Description: Staff records shall contain all the required components.
The records for staff #1-5 did not contain date of hire and emergency contacts. Staff
#2-4's records did not contain proof of ID. Staff #2,3, and 5's records did not contain the 2 required references.Plan of Correction: Center will get information for staff records.
Standard #: 8VAC20-780-240-A Description: Staff shall complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.
Staff #1, 3, 4, and 5 have not completed the Virginia Department of Education-sponsored orientation course.Plan of Correction: Center staff will complete required 10 hour VA preservice training.
