Comments:
An unannounced inspection was initiated on September 11, 2025 and completed on November 7, 2025 with a local agency in response to a complaint received by the licensing office on September 8, 2025 relating to allegations of sight and sound supervision and care and protection of the children in care. The inspector reviewed 1 child record and 4 staff records September 30, 2025 on-site. This inspection included document review, tour of the facility, interviews, and observations .

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
November 14, 2025. A POC submitted after this date will not appear on the public website.

Description: Centers shall be in compliance with the center's own policies and procedures required by these standards.

The center's policy is that teachers will always know how many children they have and who they are at any given time. Child 1 (3 years old) was left alone and unsupervised outside for approximately 10-15 minutes.

Plan of Correction: The center's Supervision and Transition Policy was reviewed, updated, and redistributed to all employees on 9/10/2025. The revised policy includes specific procedures for headcounts, transition verification, and required documentation. All staff participated in a mandatory training session reviewing these expectations. To ensure continued compliance, the policy will be reviewed with staff during onboarding, annual training, and quarterly team meetings. Administrative leadership will conduct unannounced classroom observations to verify adherence to these procedures.

Description: When staff are supervising children they shall always ensure their care, protection and guidance.

On 9/8/2025, Child 1 (3 years old) was left alone and unsupervised outside of the center playground for approximately 10-15 minutes. The center is in a residential area next to other homes and businesses. A passer-by observed the child walking alone towards the road (speed limit of 25 mph). The passer-by stopped their vehicle and then walked the child back to the program. Center staff were unaware that the child was missing.

Plan of Correction: All staff have completed a refresher training on Active Supervision, including positioning, scanning, zone coverage, and maintaining ratio awareness. Staff were instructed on immediate response protocols should a child become separated from the group. The incident was reviewed as a case study during a staff meeting to reinforce accountability and the importance of proactive supervision. Going forward, administrative staff will conduct documented supervision checks weekly for a period of 60 days, and ongoing monitoring thereafter. Any deviation from supervision policy will be addressed through corrective action and retraining as necessary.

Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 9/8/2025, Staff 1 left Child 1 (3 years old) alone, outside the center for approximately 10-15 minutes. The child was not within the actual sight and sound supervision of center staff.

Plan of Correction: Immediately following the incident, administration conducted an internal review and debrief with all involved staff to determine contributing factors. The staff were subsequently retrained on the center's Active Supervision Policy and Transition Procedures to reinforce the requirement that children must remain within both sight and sound supervision at all times. A new transition checklist was implemented requiring physical headcounts before and after outdoor play, and visual confirmation by both staff members assigned to the group. Additionally, an audible door alarm was installed n the playground exit door as an added safety measure to alert if a door is opened unexpectedly. Administrative staff will conduct random supervision audits to ensure continued compliance with all supervision standards.
