An unannounced inspection was initiated on and completed on 10/8/2025. in response to a complaint received by the licensing office on 9/30/2025 relating to allegations of Staffing and Supervision. This inspection included document review, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found and are documented on the violation notice issued to the program.
