An unannounced monitoring inspection was initiated on 07/24/2025 and completed on 07/28/2025, in response to a self-report received by the licensing office on 07/18/2025 relating to a serious injury requiring outside medical treatment. The inspector reviewed one child?s record and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
