During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on 10-7-2025 and completed on 10-14-2025. The on-site inspection began at 4:40pm and ended at 5:35pm. The inspector reviewed compliance in the areas listed above. There were 43 children present and 3 staff. The
inspector reviewed 5 children?s records on-site and 4 staff records electronically from 10-8-2025 to 10-14-2025. This inspection included document review, tour of the facility interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on
10-21-2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-245-J-3 Description: Any child for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed shall always be in the care of a staff member or independent contractor who meets the training requirements. On October 7, 2025 Child #1, has 3 emergency medications and was in care for approximately 3 hour and Child #2 was in care for approximately 2 hours and has 2 emergency medications. There was not a staff member with the required medication administration training on site on October 7, 2025 during the time Child #1 and Child #2 were in care. Plan of Correction: We have one staff member with Medication Administration Training. We are having
another staff member complete Medication Administration Training on 10.18.25
