During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site focused monitoring inspection was initiated and completed on 10/7/2025. The Department issued a Notice of Intent to Deny Renewal of the License on May 27, 2025, as a result of high-risk violations. The on-site inspection began at 11:35am and ended at 1:25pm. The inspector reviewed compliance in the areas listed above. There were 35 children present and 7 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/21/2025. A POC submitted after this date will not appear on the public website.

Areas and equipment outside the center shall be maintained in a clean, safe and operable condition.
A long black electrical cord was observed within the fenced playground area, which is directly attached to the exterior of the center. The electrical cord enters the building near the base and extends upward to the roofline. Excess slack from the cord lies loosely along the ground and is draped over the playground fence, making it accessible to children. The unsecured and slack portion of the electrical cord presents tripping, entanglement, and strangulation hazards to children playing on the playground.Plan of Correction: During the inspection the black cord was secured and placed out of reach of the children.
The electrical cord has been permanently secured and removed from the reach of children. The slack was eliminated, and the cord was professionally fastened to the building's exterior at an elevated height using appropriate clips and conduits to ensure it is no longer accessible for the children to grab.
