During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated and completed on 10/07/2025, as a part of the licensure period. The on-site inspection began at 10:30am and ended at 1:40pm. The inspector reviewed compliance in the areas listed above. There were 70 children present and 13 staff. The inspector reviewed 7 children?s records, 6 medication records, and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-350-A
Description: Maximum group size limitations shall be followed whenever children are in care. The maximum group size for ages birth-16 months is 12 children. There were 13 children in care in the infant room during the inspection.

Plan of Correction: A plan is in place to transition one of the children to the next age group, once a written assessment is completed by the director.

Standard #: 8VAC20-780-510-P
Description: When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days are required to be disposed of by the center. The authorization for a medication for Child #9 expired more than three months prior to the date of the inspection. The authorization for a medication for Child #11 expired 6 months prior to the date of the inspection. The authorization for a medication for Child #13 expired two months prior to the date of the inspection. The medications for Child #8, 10, and 12 were not picked up by a parent and were not disposed of as required.

Plan of Correction: Parents were contacted during the inspection.
