An unannounced, on-site monitoring inspection was initiated on 10/15/2025 and completed on 10/15/2025, as a part of the licensure period. The on-site inspection began at 11:00AM and ended at 1:13PM. The inspector reviewed compliance in the areas listed above. There were 80 children present and 17 staff. The inspector reviewed 5 children?s records on site on 10/15/2025 and 4 staff records on-site on 10/15/2025. Medication was reviewed on site 10/15/2025.
This inspection included - document review, tour of the facility, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
- Violations:

Standard #: 8VAC20-780-500-A Description: Children?s hands shall be washed with soap and running water after toileting, or any contact with blood, feces or urine. Staff #5 changed a child?s diaper in the 2?s room and did not wash the child's hands with soap and water. Plan of Correction: Provider will ensure staff wash children's hands.
Standard #: 8VAC20-780-500-B Description: A nonabsorbent surface for diapering or changing shall be used. The changing pads in the waddlers class were cracked along the edges and the foam was exposed. Plan of Correction: Provider replaced both changing pads.
