An unannounced, on-site renewal inspection was initiated and completed on October 20, 2025, as a part of the licensure period. The on-site inspection began at 10:20am and ended at 12:50pm. The inspector reviewed compliance in the areas listed above. There were 105 children present and 22 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-780-60-A
Description:	Repeat Violation Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain required information. The record for Child #5 did not have documentation of child updates and confirmation of up to date information in the child's record.
Plan of Correction:	The parent will review the child's emergency contact information and document the record today. Moving forward, children's records will be checked monthly to ensure information is up to date.
Standard #:	8VAC20-780-270-A
Description:	Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe ad operable condition. In the three year old classroom, there was peeling paint located on the wall under the window in the back of the classroom.
Plan of Correction:	A work order will be placed immediately to touch up the peeling paint in the classroom.
Standard #:	8VAC20-780-570-E
Description:	Repeat Violation Prepared infant formula shall be refrigerated, dated and labeled with the child's name. Two bottles in the refrigerator were not dated in the infant classroom.
Plan of Correction:	Staff will be retrained on labeling bottles completely. Teachers will check bottles at the time of arrival to ensure bottles are completely labeled.
