An unannounced renewal inspection was completed on 10/07/2025 as a part of the licensure period. The on-site inspection began at 11:00am and ended at 12:30pm.The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. There were 17 children present and 6 staff. The inspector reviewed 5 children?s records and 2 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-150-B Description: Immunizations reports shall include the date of the physical examination and dates
immunizations were received and shall be signed by a physician, his designee, or an
official of a local health department. The immunization report for child #4 was not signed by a physician, his designee, or an official of a local health department.Plan of Correction: The director will contact the parent and have them resubmit the immunization record after it has been signed by the child's physician.
Standard #: 8VAC20-780-210-A-4 Description: Staff who are program leaders are required to meet at least 1 of 4 criteria. The center qualified Staff #1 as having 6 months experience and 24 hours of training. however Staff #1 only had 20 hours of documented training. Plan of Correction: Staff #1 has almost completed all of the 24 hours of training. She should be complete by next week.
Standard #: 8VAC20-780-540-C Description: The center is required to have all first aid items in their kit. The center did not have 1 of the 2 required triangular bandages in the first aid kit. Plan of Correction: The center had 1 triangular bandages and will obtain an additional one for the first aid kit.
