Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 10/22/2025, as a part of the licensure period. The on-site inspection began at 11:30am and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 9 children present and 4 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 10/29/2025. A POC submitted after this date will not appear on the public website.

Standard #:	8VAC20-780-160-A
Description:	Each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis screening submitted at the time of employment. Staff #1 has a tuberculosis screening dated 9/20/25. Staff #1 had been employed for 10 days prior to obtaining the required screening.
Plan of Correction:	The Director will ensure that documentation of a negative tuberculosis screening is submitted at the time of employment.
Standard #:	8VAC20-780-560-F
Description:	When centers provide meals or snacks they will follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). The meal served to children in care ages 2.5 years through 4 years old did not meet USDA portion requirements for protein, grains, vegetables, fruit or milk.
Plan of Correction:	Kitchen staff was re-trained to ensure that USDA guidelines are being followed
