An unannounced monitoring inspection was initiated on 08/12/2025 and completed on 10/07/2025 in response to a complaint received by the licensing office on 07/30/2025 relating to supervision and ratio. The inspector reviewed three children?s records and four staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-340-A Complaint related: Yes Description: When staff are supervising children, they shall always ensure their care, protection, and guidance. On July 30, 2025, Classroom A was on the playground with Staff #1 and Staff #2 and ten children. Collateral witness #1 reported Child A, who has special needs and uses a walker to move, was found lying on the ground, unable to move, with a flushed face and no water at approximately 3:30pm. Staff #1 and Staff #2 received a written warning for not providing water and comfort to Child A in a timely manner. On July 30, 2025, the website https://weatherspark.com/ indicated the approximated temperature on that date and time was 90 degrees. Plan of Correction: See Intensive Plan of Correction
