Technical assistance was provided regarding labeling bottles to ensure that the appropriate bottle is given to the right infant.

An unannounced, on-site monitoring inspection was initiated and completed on 10/20/25, The on-site inspection began at 9:55 am and ended at 11:55 am. The inspector reviewed compliance in the areas listed above. There were seven children present and six staff. The inspector reviewed four children?s records on-site.

Standard #: 8VAC20-780-130-A Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child #2, who had been attending for two months, did not have documentation of an immunization record on file.Plan of Correction: Parents are notified and immunization record is in child's file.
