During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated and completed on 10/20/25, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to supervision, building maintenance, and parental involvement. The on-site inspection began at 3:10pm and ended at 3:50pm. The inspector reviewed compliance in the areas listed above. There were nine children present and three staff. The inspector reviewed five children?s records and zero staff records on-site.
This inspection included document review and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/7/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-350-C Description: Repeat Violation
When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.
At the inspector's arrival, there were nine children and only two staff members in a classroom. The youngest child present was 11 months old, requiring the ratio to be 1:4. A third staff member arrived approximately 20 minutes later.Plan of Correction: Not available online. Contact Inspector for more information.
