During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated and completed on 09/29/2025 with local agency DSS in response to a complaint received by the licensing office on 09/26/2025 relating to allegations of supervision. The inspector reviewed three children?s records and three staff records on-site.
This inspection included:

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to policies/procedures and ratios and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/10/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Complaint related: No Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures.
On 9/29/25 at approximately 11:00 a.m. the Licensing Inspector observed 12 children ages 4 -5 years old exiting the Pre-K classroom with 1 staff to the playground. The director removed two children from the group and placed them in the two year old classroom. Once the remaining ten children reached the playground, staff A did not follow the center's name to face accountability procedure to account for children when each threshold is crossed.Plan of Correction: Not available online. Standard #: 8VAC20-780-350-B-4 Complaint related: No Description: The required ratio for children age three years up to school age is one staff per ten children.
On 9/29/2025 at approximately 11:00 a.m. the Licensing Inspector observed one staff (staff A) supervising 12 children ranging in ages from 4 - 5 years old.
