During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:
An unannounced, on-site monitoring inspection was completed on 10/7/2025, as a part of the licensure period. The on-site inspection began at 9:30 am and ended at 2:00 pm The inspector reviewed compliance in the areas listed above. There were 111 children present and 20 staff. The inspector reviewed 9 children?s records and 6 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-80-B-2
Description: The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

A child was injured and required outside medical attention on 8/13/2025. No report was made to the depart as required.

Plan of Correction: Training for all administrators on reporting incident. All incidents that require outside medical attention will be reported.

Description: Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

There were 2 broken ride on bikes, 2 red bikes with rust on the playground for older children,
