During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated and completed on 10/29/2025. The on-site inspection began at 11:00 a.m. and ended at 1:30 p.m. The inspector reviewed compliance in the areas listed above. There were 21 children present and 7 staff. The inspector reviewed six children?s records and five staff records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Standard #: 8VAC20-780-150-B Description: Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department.

Standard #: 8VAC20-780-40-M Description: The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
The allergies, sensitivities, and dietary restrictions lists in each classroom are not current. 1. The children documented on the allergy list dated 8/22/25 are no longer enrolled in the program. 2. Standard #: 8VAC20-780-270-A Description: REPEAT VIOLATION
The center shall ensure that areas and equipment of the center, inside and outside, shall be maintained in safe and operable condition.
1.) Three trikes were observed to be rusted with peeling enamel.
2.) One trike seat was cracked and posed a pinching hazard.
3.) One trike seat was broken leaving sharp edges exposed.
4.) Rust was present on the metal triangle pieces attaching the swing seats to the swing chains on the school-age playground swing bay.Plan of Correction: Not available online. Contact Inspector for more information.
