An unannounced, on-site renewal inspection was completed on 09/25/2025. The inspection began at 2:32 p.m. and ended at 5:06 p.m. The inspector reviewed compliance in the areas listed above. There were 35 children present and 3 staff. The inspector reviewed 6 children?s records on-site and 3 staff records off-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/07/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled
that contains all of the required information. The record for C1 did not contain the name, address, and phone number for second designated person to call in an emergency if a parent cannot be reached.Plan of Correction: Director has contacted parents to fill in required information.
Standard #: 8VAC20-780-240-B Description: Staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities. S1 completed orientation training 25 days after assuming job responsibilities. Plan of Correction: Admin will ensure orientation checklist are done within 7 days.
Standard #: 8VAC20-780-550-P Description: The center shall maintain a written record of children?s minor injuries. During an interview with S1, S1 reported she used an antiseptic wipe on C7 for a minor scrape the previous week while the child was on the playground. S1 reported not writing an injury report. Plan of Correction: Injury has been documented.
