Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Inspector clarified with center director and district manager that staff files must have required information for each business entity/LLC that a staff works.

Comments:

An unannounced monitoring inspection was initiated on 10/24/2025 and completed on 10/30/2025 in response to a self-report of received by the licensing office on 10/20/2025 relating to a child's incident report requiring outside medical care. The inspector reviewed 3 children?s records on-site. This inspection included document review, tour of the facility, interviews, and observations. Though not a part of the original self report, violations that were not reported were found related to staff records and are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/6/2025. A POC submitted after this date will not appear on the public website.

Standard #:	22.1-289.035-B-1
Description:	SYSTEMIC DEFICIENCY The center is required to obtain a completed sworn statement prior to the employee's first day of employment. The sworn statement for Staff #1, who was working on 10/20/2025, did not have a sworn statement on file at the center.
Plan of Correction:	Prior to an employee from another KinderCare location working at the center, the management team will ensure that staff file in on site and the business entities on the center licenses match. If entities do not match, employee will complete requirements before working at the center.
Standard #:	22.1-289.035-B-2
Description:	SYSTEMIC VIOLATION The center must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #1, who was working on 10/20/2025, did not have a completed national criminal background check on file at the center.
Plan of Correction:	Prior to an employee from another KinderCare location working at the center, the management team will ensure that staff file in on site and the business entities on the center licenses match. If entities do not match, employee will complete requirements before working at the center.

Description:	SYSTEMIC DEFICIENCY The center is required to obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. Staff #1 worked at the center on 10/20/2025 and did not have TB screening on file.

Description:	SYSTEMIC DEFICIENCY Staff records shall be kept for each staff person with all required information. The center did not have any of the required information for staff #1, who was working with children on 10/20/2025.
