Staff #1 became aware of a reportable matter on March 29,2025. Child #1 stated" My father beats me" during class. When the statement of concern was reported to Staff #1, they did not report the concern to the local child protective services office or the department?s toll-free child abuse and neglect hotline.Plan of Correction: Not available online.
