An unannounced, on-site monitoring and focused inspection was initiated on 10/22/2025 and completed on 10/22/2025, an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to 400-B. The on-site inspection began at 1:21 pm and ended at 3:55 pm. The inspector reviewed compliance in the areas listed above.There were 55 children present and 14 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued
