An unannounced inspection was initiated and completed on 11/03/2025 in response to a complaint received by the licensing office on 10/31/2025 relating to allegations of transportation.
This inspection included one interview.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation is documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 11/10/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-580-B Complaint related: No Description: Any vehicle used by the center for the transportation of children shall meet the safety standards set by the Department of Motor Vehicles and shall be kept in satisfactory condition to assure the safety of children.
Vehicles that transport children are required to have a current registration with current license plate tags.Staff stated Bus#1 had an expired license plate that expired July 2025.Plan of Correction: Bus was taken to get emissions on 11/0/2025. Bus was dropped off for all testing today. 11/04/2025
