During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If
there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Discussed safe sleep and infant cribs.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 10/27/2025. The on-site inspection began at 3:10pm and ended at 4:20pm. The inspector reviewed compliance in the areas listed above. There were 44 children present and 10 staff. The inspector reviewed 5
children?s records and 4 staff records on-site on 10/27/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: All employees are required to undergo a background check in accordance with subsection B prior to employment and every five years thereafter.

Staff #3's sworn statement expired over one month ago.
Staff #3's central registry background check expired over one month ago.
Staff #3's fingerprints results expired over one month ago.

Plan of Correction: We will start the process for updating the background checks today and complete within the required timeframe.

Standard #: 22.1-289.035-B-4
Description: The Child day center is required to obtain a copy of the results of a criminal history record
information check, a sex offender registry check, and a search of the child abuse and
neglect registry or equivalent registry from any state in which an individual (employee) has
resided in the preceding five years.

Staff #2 did not have results of an out-of-state Central registry check and sex offender registry
check for a state in which the employee has lived in the preceding 5 years and has been
employed for 9 months.

Plan of Correction: We will start the process for completing the out-of-state background checks today and complete within the required timeframe.

Standard #: 8VAC20-780-130-E
Description: The center is required to obtain documentation of additional immunizations once every six months for children under the age of two years.

Child #2's last update of immunizations was in April 2025.
Child #5's last update of immunizations was in October 2023.

Plan of Correction: We will request the updated immunizations from the parents.

Standard #: 8VAC20-780-160-C
Description: At least every two years from the date of the first initial screening or testing staff are required to obtain and submit the results of a follow-up tuberculosis screening.

Staff #4's most recent TB screening on file expired over one year ago.

Plan of Correction: We will ask the staff to get a new TB screening.
