During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 11/05/2025, as a part of the licensure period. The on-site inspection began at 11:05 AM and ended at 12:50 PM. The inspector reviewed compliance in the areas listed above. There were 62 children present and 17 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 11/13/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-B-1
Description: The center is required to obtain a completed sworn statement prior to the employee's first day of employment.
The sworn statement for Staff B, who is currently working, was dated two days after their first day of employment.

Plan of Correction: The Sworn Statement was signed and completed during the inspection.

Standard #: 8VAC20-780-60-A-8
Description: A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.
Child #2, who has a diagnosed food allergy, did not have a written care plan in their file.

Plan of Correction: The family was contacted during the inspection and submitted paperwork to the doctor for them to return.

Standard #: 8VAC20-780-510-F
Description: The medication authorization shall be available to staff during the entire time it is effective.
Child #2 had a medication at the center without a medication authorization. The authorization available authorized Fairfax County Schools to administer the medicine.

Plan of Correction: At pickup, the parent completed and signed a VDOW medication authorization form until the doctor returns this within 10 working days.

Standard #: 8VAC20-780-510-I
Description: The center administers only those drugs that were dispensed from a pharmacy and maintained in the original, labeled container.
Child #1's prescription medication did not have a prescription label attached to the container.

Plan of Correction: At pickup, the parent dropped off the same prescription with the label.
