During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. 8VAC20-780-270A Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
The Provider has decided not to use the two pieces of equipment in question, and they plan to block off this equipment until a determination is made on its safety or until they explore the possibility of an Allowable Variance.

An announced, on-site focused inspection was initiated and completed on 11-6-25 as the result of a request by the Provider to inspect the playground. The on-site inspection began at 9:40am and ended at 10:30am. The inspectors reviewed compliance in the areas listed above. There were no children present during the inspection, but there were administrative staff present. The playground has not been in use since 10-7-25. This inspection included a tour of the playground facility, interviews, observations, and measurements. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
