During the inspection, the licensing representatives reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated and completed on 10/29/2025. The on-site inspection began at 6:40 AM and ended at 8:00 AM. The inspectors reviewed compliance in the areas listed above. There were 29 children present and 4 staff. The inspectors reviewed 5 children?s records, 3 medical records, and 4 staff records on-site. This inspection included document review, interviews, and a tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standard or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
The center did not request a search of the child abuse and neglect registry from one state for Staff #1, who has been employed 2 years and was working with children at the time of the inspection.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-245-J-3 Description: Any child for whom emergency medications have been prescribed shall always be in the care of a staff member who meets the requirements of medication administration training.
On 10/29/2025 for 12 minutes, Child #5, who has been prescribed emergency medication, was not in the care of a staff member that meets the requirements of medication administration training.Plan of Correction: Two additional staff are getting MAT trained on 11/13/2025
