During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated on 11/6/2025 and completed on 11/6/2025. The on-site inspection began at 10:15 am and ended at 12:35 pm. The inspector reviewed compliance in the areas listed above. There were 39 children present and 14 staff. The inspector reviewed 5 children?s records and 6 staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/14/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-160-A
Description: Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #2, who had been employed for two months, did not have a TB screening completed within 30 days of their employment start date.

Plan of Correction: Staff #2 completed TB

Standard #: 8VAC20-780-160-C
Description: A tuberculosis (TB) screening is required every two years from the last screening.

Staff #1?s updated screening is overdue by one month.

Plan of Correction: Staff #1 went and completed the TB test

Standard #: 8VAC20-780-510-P
Description: When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

At the time of the inspection, the medication authorization forms for two of Child #3?s onsite medications had expired.

Plan of Correction: Parent has been notified that medication authorization for child #3 has expired. Parent will provide updated authorization and new medications.
