During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated and completed on November 7, 2025. The on-site inspection began at 9:40am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 97 children present and 14 staff. The inspector reviewed 9 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 14, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-60-A Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
The record for child #2 did not contain name, address, and phone number of two designated people to call in an emergency.Plan of Correction: Parent has provided the missing emergency contacts for the file. Going forward we will ensure emergency contacts are provided upon enrollment.
Standard #: 8VAC20-780-270-A Description: The center shall ensure that areas and equipment of the center, inside, are maintained in a clean, safe and operable condition.
There was a small tear in the changing pad in classroom #1. Chipped and peeling paint were observed in classroom #2, #3, and #4.Plan of Correction: Changing pad was removed the same day and disposed. Replacement ordered. Work orders have been entered for painting to be completed.
Standard #: 8VAC20-780-290-A-3 Description: In areas used by preschool age or younger children, electrical outlets shall have protective covers.
Two electrical outlets were observed without protective covers in classroom #3 (age: 3 year old's).Plan of Correction: Outlets were covered. We will remind cleaning staff and regular staff to check outlets regularly to make sure covers are on.
Standard #: 8VAC20-780-500-A Description: Children's hands shall be washed with soap and running water after toileting and any other contact with blood, feces, or urine.
Staff #1 did not wash child #1 (age: 16 months) hands after changing the child?s diaper.Plan of Correction: We will conduct retraining with the staff member on hand washing standards.
