22.1 BACKGROUND CHECKS, CODE, CARBON MONOXIDE

During the inspection, the licensing representatives reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.?

Comments:
An unannounced, onsite monitoring inspection was initiated and completed on 11/10/2025. The on-site inspection began at 11:00 am and ended at 12:50 pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and 6 staff. The inspector reviewed 5 children?s records and 3 staff records.

The inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-2
Description: Providers must obtain a completed national criminal background check prior to employment.

There was not a completed national criminal background check for Staff 2 available during the inspection; Staff 2 was working during the inspection.

Plan of Correction: The fingerprints have been done. Moving forward, will not hire before the results are received.

Standard #: 8VAC20-780-520-A
Description: All over-the-counter skin products shall not be kept or used beyond the expiration date of the product.

There were 5 over-the-counter skin products on-site that were expired.

Plan of Correction: Will send them home or throw them away.

Standard #: 8VAC20-780-560-F-4
Description: Children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

The children in Classrooms 2, 3 and 4, ages 15 months-4 years, were served mini corndog nuggets that were not cut/sliced in a way to prevent the hotdog inside of the breading from being a potential choking hazard.
