Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, onsite monitoring inspection was initiated and completed on 11/10/2025. The on-site inspection began at 11:00 am and ended at 12:50 pm. The inspector reviewed compliance in the areas listed above. There were 17 children present and 6 staff. The inspector reviewed 5 children?s records and 3 staff records. The inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #:	22.1-289.035-B-2
Description:	Providers must obtain a completed national criminal background check prior to employment. There was not a completed national criminal background check for Staff 2 available during the inspection; Staff 2 was working during the inspection.
Plan of Correction:	The fingerprints have been done. Moving forward, will not hire before the results are received.
Standard #:	8VAC20-780-520-A
Description:	All over-the-counter skin products shall not be kept or used beyond the expiration date of the product. There were 5 over-the-counter skin products on-site that were expired.
Plan of Correction:	Will send them home or throw them away.
Standard #:	8VAC20-780-560-F-4
Description:	Children three years of age or younger may not be offered foods that are considered to be potential choking hazards. The children in Classrooms 2, 3 and 4, ages 15 months-4 years, were served mini corndog nuggets that were not cut/sliced in a way to prevent the hotdog inside of the breading from being a potential choking hazard.
