During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
* HEALTH INSPECTIONS
In collaboration with the Virginia Department of Health (VDH), the VDOE is reminding providers that the Food Establishment Health form is only used by VDH when determining compliance with food regulations.

A Report of Environmental Sanitation Inspection form should be used for environmental health inspections and determining compliance with non-food related health requirements such as water supply and sewage disposal. The Report of Environmental Sanitation Inspection Form can be found on the Department?s website.

Please request that your Health Inspector provide BOTH reports if your program provides food service.

* WATER TESTING
Effective January 1, 2025, the VDOE began determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site renewal inspection was initiated and completed on 10/28/2025. The on-site inspection began at 2:40 pm and ended at 5:10 pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site on This in11/28/2025. This inspection included document review, tour of the facility, interviews, observations, measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-160-C
Description: (Repeat Violation) At least every two years from the date of the first initial screening, staff members must obtain and submit the results of a follow-up tuberculosis screening.
The most recent documentation of TB screening for staff #1 is more than 2 months overdue. The TB screening for staff #2 is more then 5 months overdue.

Plan of Correction: The program director will speak to staff #1 and staff #2 today and request that they have updated TB screenings updated within 2 weeks.

Standard #: 8VAC20-780-350-B-1
Description: The required ratio of adult to children for children birth to 6 months old is 1 staff per 4 children.
There were six infants, ages 2 months to 1 year old with one staff in the infant room.

Plan of Correction: A second staff had been in the classroom but left briefly to handle a personal task. The staff was gone approximately 20 minutes. There were adequate staff in other classrooms to cover the ratios. In the future, staff will use the hand held radios to communicate with each other and the staff in charge and determine if another staff, not required for their group's ratios, can assist.

Standard #: 8VAC20-780-560-F
Description: When centers choose to provide meals or snacks, they must follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
The children were observed eating an afternoon snack which consisted of animal crackers and water, which only meets one food component and does not meet the requirements of USDA.

Plan of Correction: The director will remind staff to follow the posted monthly menu and to call for assistance if the correct food is not brought to their classroom for meals or snacks.
