During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on 11/10/25 and completed on the same day, as a part of licensure period. The on-site inspection began at 11:45 and ended at 13:30. The inspector reviewed compliance in the areas listed above. There were 19 children present with 3 staff in two groupings.. The inspector reviewed 5 children?s records and 3 teachers records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-A
Description: Background checks must be redone every 5 years.

Staff 1 and staff 3 are missing subsequent background checks.

Standard #: 8VAC20-770-60-C-2
Description: The licensed or registered child day program or family day system does not have a central registry finding within 30 days of employment or volunteer service.

One staff, staff 2, are missing the central registry letter in the file.

Standard #: 8VAC20-780-160-C
Description: At least every two years from the date of the first initial screening or testing, or more frequently as if recommended by a licensed physician or the local health department, staff members and individuals from independent contractors shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section.

Description: Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information emergency contacts addresses and parents work addresses.

One child, child 1 is missing the addresses for parents work as well as addresses for the emergency contacts.

One child, child 5, does not have a record at all, this is his first day.

Description: When food is brought from home, the food must be identified with the name of the child and the date.

7 out of 8 lunchboxes were not labeled with name and date.
Bottle for a toddler was also not labeled.
