During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on November 7, 2025 and completed on November 13, 2025. The on-site inspection began at 11:10 am and ended at 2:10 pm. The inspector reviewed compliance in the areas listed above. There were 132 children present and 27 staff. The inspector reviewed 9 children?s records and 3 staff records on-site on November 7, 2025.
This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 20, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-D Description: Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. Staff #1 and Staff #2, who did not meet lead teacher qualifications. Plan of Correction: We have obtained the missing documentation, including diplomas verifying their level of
education, and all items have been added to their staff files.
Standard #: 8VAC20-780-510-G Description: Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. There was no medication label with the required information for child A. Plan of Correction: We have received all missing information from the child?s guardian, and all required items have now been properly labeled and dated
Standard #: 8VAC20-780-510-I Description: Prescription medication may be administered if the parent has completed a written authorization from a parent or guardian. There was no written parent/guardian or physician authorization to administer prescription medications to child B. Plan of Correction: We have obtained the parent authorization documents to provide medication, and the forms are on file and in the classrooms
