Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Health Inspections - In collaboration with the Virginia Department of Health (VDH), the VDOE is reminding providers that the Food Establishment Health form is only used by VDH when determining compliance with food regulations. A Report of Environmental Sanitation Inspection form should be used for environmental health inspections and determining compliance with non-food related health requirements such as water supply and sewage disposal. The Report of Environmental Sanitation Inspection form can be found on the Department?s website.

Comments:

An unannounced, on-site renewal inspection was initiated and completed on November 5, 2025. The on-site inspection began at 9:30 am and ended at 11:20 am. The inspector reviewed compliance in the areas listed above. There were twenty-seven children present and seven staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:

Standard #:	8VAC20-780-80-A
Description:	(Repeat violation) For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. There were seven children in the preschool room, but only five were marked present on the attendance record.
Plan of Correction:	Director will have a meeting with staff to reiterate staff are to ensure children are signed in and out when moved to another classroom.
Standard #:	8VAC20-780-530-A
Description:	At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid. Staff #3 was alone with a group of toddler/2's children and was not certified in CPR and first aid.
Plan of Correction:	Staff is scheduled for CPR/first aid class.
