During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 11/05/2025, as a part of the licensure period. The on-site inspection began at 3:20 PM and ended at 4:30 PM. The inspector reviewed compliance in the areas listed above. There were 20 children present and 4 staff. The inspector reviewed 8 children?s records and 5 staff records on-site.
This inspection included:

? interviews, and
? observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on November 21, 2025. A POC submitted after this date will not appear on the public website.

Description: Repeat violation
Each center shall maintain and keep at the center a separate record for each child enrolled that includes the names, addresses and telephone numbers for each parent.
The record provided for child 4 did not include any information regarding the father.

Description: Repeat violation
The ratio of children to staff in a mixed age group of children aged four years through school age is not more than ten children per staff member.
At 3:55 PM there were 29 children ages four years through school age with one staff. at 3:58 PM a second staff person arrived. At 4:01 PM a third staff member arrived and at 4:04 PM the fourth required staff member arrived. Out of child to staff ratio for 9 minutes.

Description: Repeat violation
Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills that includes all required information.
The documentation of evacuation drills conducted did not include the time the drill was conducted, the method used for notification of the drill, the number of staff present during the drill and the weather conditions at the time of the drill.

Description: A menu listing foods to be served for meals and snacks during the current one-week period shall be posted in a location conspicuous to parents.
The posted menu was for the month of October. Staff stated that they had not yet been given a menu for November.

Description: Tables shall be sanitized before and after each use for eating.
The tables were cleaned using a cloth and a bucket of solution after the children ate dinner.
