An unannounced inspection was initiated and completed on 11/19/2025 in response to a complaint received by the licensing office on 11/18/2025 relating to allegations of forbidden actions. The inspector reviewed one child's recordsand one staff record [on-site on 11/19/2025. This inspection included document review, observations and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to children's reocrds and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 11/26/2025. A POC submitted after this date will not appear on the public website.

There was no physical examination in the record of child #1, who had been enrolled for approximately 4 1/2 months.Plan of Correction: Child has paused enrolled until documents are turned into school.
