Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 10/10/25. The on-site inspection began at 7:07 am and ended at 8:10 am. The inspector reviewed compliance in the areas listed above. There were 35 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/26/25 . A POC submitted after this date will not appear on the public website.

Description:	A child day center must obtain a copy of the results of a sex offender registry check from any state in which the individual has resided in the preceding five years prior to employment. There was no documentation of an out of state sex offender name search for staff A. This background check was overdue by one month.

Description:	The center shall obtain documentation that each child has received immunizations required by the State Board of Health before the child can attend the center. There was no documentation of immunization record on file for child D. This information was overdue by 6 weeks.

Description:	Each child shall have a physical examination by or under the direction of a physician within 30 days of enrollment. There was no documentation of a physical on file for child D. The documentation was overdue by 2 weeks.

Description:	The center's procedures for administering medication shall include duration of the parents authorization for medication, provided that it shall expire or be renewed after 10 work days. Long term prescription drug use and over the counter medication may be allowed with written authorization from the child's physician and parent. Child A's parental authorization for an emergency medication expired five weeks prior. . Child B's parental authorization for a medication had not been renewed every 10 work days nor was their a physician authorization. The parent authorization had expired a month earlier. Child C's parental authorization for a medication had not been renewed every 10 work days nor was their a physician authorization. The parent authorization had expired a month earlier.
