On 11/19/2020 a virtual Subsidy Health and Safety inspection was conducted. Due to the COVID 19 pandemic the inspector was not on site. This inspection was conducted via mobile video application, email and phone correspondence. During this inspection there were no children present. Emergency supplies, procedures and drills were reviewed. The home and facilities were inspected via video mobile application. In addition, Child, provider and household member records were reviewed. Violations are indicated in the violation notice.

Standard #: 63.2-1721.1-E Description: Based on review of household member records and conversation with vendor, the fingerprints for household member 1 were unavailable.
Evidence: Vendor did not have fingerprints for household member who'd recently turned 18Plan of Correction: Vendor stated that background checks were underway via local authorities, local authorities were contacted and confirmed.
