During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated on November 10, 2025 and completed on November 17, 2025 in response to a self-report received by the licensing office on November 7, 2025 relating to behavioral guidance and forbidden actions. The inspector reviewed one child?s record and one staff record on-site. Additional documentation was received on November 17, 2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 1, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards.
1. The center?s discipline policy states that ?The following actions or threats; therefore, are forbidden: Physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume and un-comfortable position or exercise as punishment".
2. On November 6, 2025, video footage showed staff #1 grabbed child #1 (2 years old) under the arm, and aggressively put him on the ground in response to the child attempting to bit staff #1.Plan of Correction: Advent did everything per our policy. We do not tolerate rough handling of a child. The was reported to Administration immediately when it happened by another staff member. That same afternoon follow out investigation and review of our camera footage, we spoke with the employee and she was fired immediately. This happened at the end of the day on 11/06/25. I reported to the mother of child involved at 6 PM on 11/06/25. Reported to VDSS on 11/06/25 and to VDOE on 11/07/25.
Standard #: 8VAC20-780-400-B Description: Behavior guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflict.
On November 6, 2025, video footage showed two children trying to sit on the same see-saw seat. Child #1 (2 years old) sat down first and staff #1 came over and began holding down the see-saw so that it would not move. Child #1 continued to try and move the see-saw while staff #1 had both hands on the handlebars. Child #1 attempted to bit staff #1?s right hand and staff #1 threw her arm up, grabbed child #1 under the arm, and aggressively put him on the ground.Plan of Correction: Advent did everything per our policy. On November 6, 2025, video footage showed staff #1 on the playground. Staff #1 was observed picking up child #1 (2 years old) by one arm and aggressively putting him down on the ground. Video footage showed that child #1 was laying face down on the ground when staff #1 let go of his arm.Plan of Correction: Advent does not tolerate rough handling of a child. The employee was fired immediately.
